OTSD end of year inventory sheet instructions
Attached to this email is the end of year computer inventory form that you will need to fill out completely and have signed by your building principal by the final teacher day. You will need Adobe Reader to fill out this form, if you do not have adobe reader please go to www.adobe.com to download it. Once you have the form opened you can easily type the required information into the boxes then click the “Print Form” button or select “File” then “Print” to print out the form so that it can be signed and turned in. For LMS teachers with MLTI laptops; you do not need to fill out this form for the MLTI laptops. Please fill out this form for the PC’s in your room. You will need to turn in your laptops at the end of the school year, unless prior arrangements have been made with I.T.
To find information on your computer name, Memory and CPU speed right click on “My Computer” and click on “Properties”. The memory and CPU info will be under the General Tab and the Computer name is under the Computer Name tab. If you have a computer running Windows NT you may not be able to find the Computer name, and may leave that blank.
Please list your preference of computer type, either desktop or laptop, in the space provided. While we cannot guarantee that you will get your preference, we will try to accommodate as best we can.
Please indicate if you would like your computer re-imaged or not, (everything will be erased and the operating system reloaded if necessary.) If you do not want it re-imaged please explain why; does it have special software or hardware etc.
If there are any know issues with your computer or if you have any special requests please let us know in the spaces provided.
If you have any information on your computer that you do not want to lose please back it up! You can place it on the Z drive but it will also be wise to burn it to a CD and/or move it to a thumb drive incase there is an issue with the Z drive over the summer.
