• The Pupil Evaluation Team meeting is held and recommendation for placement is made. The following information will be required and must be received by the Old Town Regional Program before enrollment:

• A recent psychological evaluation (within six months)
• Recent academic evaluations
• Statement of need describing problems being encountered
• A copy of current Behavior Intervention Plan
• Health Record
• Up-To-Date Transcript
• Current IEP

 

• Parent/guardian and student school visitation.  During the visit the following shall occur:

• Meeting with Director or his/her designee to discuss the program and answer any questions
• Tour the school building
• Introduction to Classroom Teacher and Social Worker
• Receive copies of the Parent/Student Handbook, Behavioral Skills Inventory and The Protocol for Responding to Assaultive and Destructive Behavior.
• Receive registration material (if placement has already been finalized at a PET meeting)

 

• Student is enrolled
